By law, an employee or administrator of a facility who has reason to suspect that a recipient is a victim of abuse or neglect shall make an oral report to the local adult protective services agency. At UCP it is required suspected abuse be reported immediately to your supervisor. As an employee, acknowledge that you understand you are now a Mandated Reporter which requires you to report suspected abuse by checking YES below.

2. By acknowledging YES below I acknowledge thorough review of the Adult Protective Services & Older Adults Protective Services Slides and I agree to report all suspected neglect or abuse to my supervisor immediately.
